Completing Paperwork
The valuable information we gather during your first visit becomes a permanent part of your dental record. But nobody enjoys paperwork, so we’ve made it as easy as possible for you by giving you the option of filling out your New Client Form online. Please reach us at (8130 642-8885 to receive our New Patient Forms.
In addition to completing your New Client Form, please bring with you a photo ID and any dental insurance cards that apply. We may ask for other information like previous x-rays and any medications you currently take, but we will explain what’s needed when you call to arrange your appointment.
Arriving at Our Office
When you arrive at A Family Dentist, you will be greeted warmly by a member of our front office team who will take your information and get you settled in our reception area.
Meeting the Team
At your appointed time, a member of our team will take you back to your treatment room to get you settled. Next, you will meet Dr. Nguyen or Dr. Anthony who will review your records, ask about your concerns, and explain what needs to be done to get your smile back to full health.
After you and your doctor develop a treatment plan, our staff will review the treatment plan and fees associated. If you have dental insurance, we could help you maximize your benefits.
